29 CFR 1926.451(g)(4)(xv):   Crossbracing used in place of a toprail was not acceptable because the crossing point of two braces is not between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart.   a) On scaffold: Employees were loading tools and materials through a scaffold frame that was missing a top rail being exposed to a fall of approximately 80 feet to a concrete floor, on or about 12/6/2012.
