On December 4, 2012, an employee was permitted to take grounded conductors and the Altec Model AM855 aerial lift, serial number 0706BB4108 closer than the minimum working distace of 2.0 feet from energized conductors.The employee was not wearing insulated personal protective equipment, and the energized conductors were not insulated or guarded, exposing the employee to electrocution hazards.
