29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  On or about June 5, 2019 employees performing blasting operations inside of tanks were exposed to atmospheric hazards because employees were not informed that they were entering a confined space through the use of danger signs.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  On or about June 5, 2019 employees using scaffolds to blast the inside of tanks were exposed to struck-by hazards when the scaffolds were not erected under the supervision and direction of a competent person.
