29 CFR 1910.137(b)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests in accordance with Tables I-5 and I-6: The employer does not perform periodic inspections on electrical protective equipment. This violation occurred on or about December 12, 2012, in the facility where employees were exposed to electrical shock hazards up to 440 volts when using Class 2 Type 1 rubber electrical gloves that were last tested in March 1998. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that electrical protective equipment is subjected to periodic electrical tests in accordance with Tables I-5 and I-6.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: The employer does not provide suitable facilities for quick drenching or flushing of the eyes and body within the work area for immediate use. This violation was observed on December 12, 2012, in the battery charging area where employees were exposed to a chemical hazard when an emergency eyewash was not functional do to the water supply being shut off at an unmarked control valve. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that suitable facilities for quick drenching or flushing of the eyes and body within the work area are provided for immediate use and that prevents unauthorized shutoff.
