Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about 12/03/2012, while installing rubber roofing material on a flat roof, employees were not protected from a fall hazard of (approximately) 30-32 feet, by a fall protection system/device or monitoring system.
