29 CFR 1910.27(d)(2): When ladders are used to ascend to heights exceeding 20 feet (except on chimneys), landing platforms were not provided for each 30 feet of height or fraction thereof: (a) On or about 12/13/2012, at establishment, for employees required to use a caged ladder system attached to the back side of the "storage" building. The heigth between the lower ladder landing platform and the upper ladder landing platform was approximately 73 feet. There was no platform installed between these two sections of ladder.

29 CFR 1910.272(o)(1): The employer did not provide at least two means of emergency escape from galleries (bin decks): (a) On or about 12/13/2012, at establishment's Buffalo tank building, for employees working or walking on bin decks that surrounded the storage tanks. There was only one means to exit from each bin deck platform above the first level.
