29 CFR 1910.134(g)(1)(i): The employer permitted respirators with tight-fitting facepieces to be worn by employees having facial hair that came between the sealing surface of the facepiece and the face:   On or about February 01, 2013, an employee was using a tight-fitting respirators, such as but not limited to, Binks half mask respirators, with facial hair, which interfered with respirator's seal and exposed the employee to inhalation hazards.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.   On or about February 01, 2013, the employer did not ensure defective forklifts were removed from service. Employees operating the two Hyster powered industrial trucks having oil leaks and torn and tattered seats were exposed to struck-by and driving hazards.

29 CFR 1910.178(m)(3): Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized.    On or about February 01, 2013 the employer permitted unauthorized personnel to ride on powered industrial trucks while in operation. Unauthorized employees riding the two Hyster powered industrial trucks were exposed to struck-by hazards.

Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10° twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section. 29 CFR 1910.179(m)(1): Running ropes. A thorough inspection of all ropes shall be made at least once a month and a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected shall be kept on file where readily available to appointed personnel. Any deterioration, resulting in appreciable loss of original strength, shall be carefully observed and determination made as to whether further use of the rope would constitute a safety hazard. Some of the conditions that could result in an appreciable loss of strength are the following:     On or about February 01, 2013 the employer did not ensure overhead hoists having ropes were inspected. Welding employees operating overhead hoists with defective ropes were exposed to struck-by hazards.

29 CFR 1910.213(i)(1): All portions of the saw blade shall be enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table. Bandsaw wheels shall be fully encased. The outside periphery of the enclosure shall be solid. The front and back of the band wheels shall be either enclosed by solid material or by wire mesh or perforated metal. Such mesh or perforated metal shall be not less than 0.037 inch (U.S. Gage No. 20), and the openings shall be not greater than three-eighths inch. Solid material used for this purpose shall be of an equivalent strength and firmness. The guard for the portion of the blade between the sliding guide and the upper-saw-wheel guard shall protect the saw blade at the front and outer side. This portion of the guard shall be self-adjusting to raise and lower with the guide. The upper-wheel guard shall be made to conform to the travel of the saw on the wheel.   On or about February 01, 2013, the employer did not ensure unused portions of a saw blade on a home-made bandsaw were guarded. Employees using the bandsaw were exposed to amputation hazards.

On or about February 01, 2013, the employer did not ensure unused openings in electrical boxes were effectively closed. Employees using electric welding equipment were exposed to electrical shock hazards.
