29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: (a) 15 Elwood St Springfield, MA: On or about December 14, 2012, an employee was observed using an extension ladder that did not extend 3 feet past the roof level he was accessing to perform his work at this residence as required by this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: (a) 15 Elwood St Springfield, MA: On or about December 14, 2012, two employees were observed roofing this residence more than 20 feet above the ground below without any fall protection as required by this subpart: LaCross Construction was previously cited for a violation(s) of a substantially similar or equivalent occupational health and safety standard, 29 CFR 1926.501(B)(11) which was contained in OSHA inspection number 657918 which became final order on October 29, 2012 with respect to a violation at 10 Allen St in Springfield, MA. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
