29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: On or about 12-13-12 at 91 Isle of Venice, Fort Lauderdale FL, 33301,an employee, who was operating an aerial lift at a height of approximately 12 feet from ground level was not trained on the proper use of the machine. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.503(a)(2)(iii): The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; On or about 12/13/12 at 91 Isle of Venice, Fort Lauderdale FL, 33301, an employee was not trained in the use of a personal fall arrest system (The employee was wearing a fall protection harness but did not have a lanyard to tie off with.) The employee was exposed to falls of approximately 12 feet to the ground. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
