29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    The employer allows employees to be exposed to a fall hazard created by performing work activity from the roof without properly utilizing fall protection systems.  The most recent occurrence was at 8826 Tudor Avenue, Overland, MO, 63114, where workers were observed working on top of the 3:12 pitch roof, approximately 19.9 feet above ground, without the use of an adequate fall protection system and/or fall arrest system.      K-MAC ROOFING, were previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 464978 citation number 1, item number 2, issued on 06/14/2012 and was affirmed as a final order on 07/05/2012, with respect to a workplace located at 3722 AV DE PARIS Drive, Florissant, MO 63034.
