29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. On 6/13/2014, workers from K-Mac Roofing, Inc. were repairing a roof of a commercial building located at 9295 Mexico Road, O Fallon MO. Two foremen and one employee were not wearing eye protection while using pneumatic nailers. K-MAC ROOFING, were previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926. 102(a)(1), which was contained in OSHA inspection number 464978 citation number 1, item number 1, issued on 06/14/2012 and was affirmed as a final order on 07/05/2012, with respect to a workplace located at 3722 AV DE PARIS Drive, Florissant, MO 63034.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On 6/13/2014, workers from K-Mac Roofing, Inc. were repairing a roof of a commercial building located at 9295 Mexico Road, O Fallon MO. Two foremen and four employees were repairing the 4 to 12 pitch roof, which was approximately eight feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. K-MAC ROOFING, was previously cited for a violation of this occupational safety and health standard 29 or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 788682 citation number 2, item number 1, issued on 12/28/2012 and was affirmed as a final order on 1/09/2013, with respect to a workplace located at 8826 Tudor Avenue, Overland , MO 63114.
