29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: (a) On December 13, 2012 an employee fell down a set of metal stairs that resulted in a bleeding cut to the employee's head. The employee was not provided transportion to a hospital and persons trained and certified in first aid were not available on site. The fall resulted in a laceration to the head that required medical staples to close the wound. The employee sought initial medical treatment on his own approximately one hour after the accident as determined on December 17, 2012. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter: (a) Employees were not trained on an annual basis on the use of a fire extinguisher and the hazards of incipient stage fire fighting as determined on December 18, 2012. ABATEMENT CERTIFICATION REQUIRED ABATEMENT NOTE: The employer can comply with the above regulation by implementing a written emergency action plan per 29 CFR 1910.38(c) and instructing all employees to not use fire extinguishers.
