29 CFR 1910.151(b):     There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:      (a)  On December 13, 2012 an employee fell down a set of metal stairs that resulted in a bleeding cut to the employee's head.  The employee was not provided transportion to a hospital  and persons trained and certified in first aid were not available on site.  The fall resulted in a laceration to the head that required medical staples to close the wound.  The employee sought initial medical treatment on his own approximately one hour after the accident as determined on December 17, 2012.    ABATEMENT CERTIFICATION REQUIRED
