A 100-foot yellow electrical extension cord (SJTW 12X3 AWG) used to power a Worm Drive Skilsaw used by an employee had bare conductors exposed. 2. An inspection of the 7 1/4-inch Worm Drive Skilsaw used by an employee revealed insulation and strain relief damage.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about October 18, 2012, two employees observed walking and working on top the outer edge of the wooden trusses without any form of fall protection. The employees were exposed to a fall hazard of approximately 11-feet to the ground. DIVISION 6 SYSTEMS LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD STANDARD 29 CFR 1926.501 (B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313497299, CITATION NUMBER 01, ITEM NUMBER 003 AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 16, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 3700 HUCEO VALLEY DRIVE, EL PASO, TX 79938.
