29 CFR 1926.307(f)(3): The employer failed to guard gears, sprockets and/or chains that were less than 7 feet (2.128 m) from the floor or platform: a) At the site, on December 18, 2012, where the employer did not ensure that there was a guard installed over the chain and sprocket of the Grout-A-Matic/Placer machine, serial #1S9BA19G18P616019. This condition exposed the employee to a caught-in hazard.
