The control room and break room were located in close proximity to the extraction process and the employer had not evaluated the structure's design capabilities to protect employees from hazards associated with building collapse and damage due to explosion overpressure, which could result in serious or fatal injuries to the building occupants.

29 CFR 1910.119(o)(1): The employer did not certify that they evaluated compliance with the provisions of this section at least every three years to verify that the procedures and practices developed under the standard were adequate and were being followed. (a) Cargill Inc., 2400 Industrial Drive, Sidney, Ohio: On or about December 19, 2012, the employer did not ensure that the Process Safety Management (PSM) program, including but not limited to, relief systems, piping and instrument diagrams (P&ID), process safety information (PSI), and process hazard analysis (PHA), was audited to ensure compliance with the OSHA standards. An audit of the PSM program did not identify non-conformances with the standard including, but not limited to, incorrect relief device information on the P&IDs, PSI was not compiled for all relief device, and the PHA did not address all the hazards of the process.
