29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems or personal fall arrest systems:    On or about October 20, 2012, on a flat roof of a two story residence, two employees were working without any form of fall protection. This condition exposed the employees to a fall hazard of approximately 24 feet to the ground level.        HP CONSTRUCTION WAS PRREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313500209, CITATION NUMBER 01 , ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 17, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 1937 SHREYA IN EL PASO, TX.

29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970:    On or about October 20, 2012, next to the tar pot at 3180 Hidden Creek, the employees were using a discharged 2A10BC chemical fire extinguisher for fire protection. This condition exposed the employee to a potential fire hazard.
