On or about December 12, 2012:  An employee engaged in truck backing and spotting activities was exposed to struck-by hazards in that Personal Protective Equipment to include high visibility vests were not provided while directing traffic at the worksite.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present.    On or about December 12, 2012:  An employee engaged in truck backing and spotting activities was exposed to struck-by hazards in that a hazard assessment had not been completed to identify the need for personal protective equipment for employees who direct traffic at the worksite.
