29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: Employees working the rig located at SEC 5, TWP 32, R 1, 958 fnl, 419 fwl, Wellington, KS 67152, were exposed to fall hazards of approximately 7 feet from the drilling floor to the ground below in that there was a 10 inch gap between the stair rail and the guardrail.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: Employees working the drilling rig located at SEC 5, TWP 32, R1, 958 fnl, 419 fwl, Wellington, KS 67152 were exposed to fall hazards of up to approximately 59 inches from the mud tank platform to the bottom of the mud tanks, in that the chains used as top rails around the mud tank openings ranged from 21 to 32 inches from the platform to the chain.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Employees working the rig located at SEC 5, TWP 32, R 1, 958 fnl, 419, fwl, Wellington, KS 67152, were exposed to electric shock hazards in that the flexible cord supplying power from the generator to the rig was used with visible damage to the outer jacket.
