29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees for safe egress/access: (a) Park Range Construction, Inc. On or before December 13, 2012, the exposing employer did not ensure that a ladder, ramp or other safe means of egress was provided for the employees working in the vertically-walled trench, which measured 4 1 deep, 26 wide. The means of entry and egress were ascending and descending the trench end and climbing up the vertically-sided walls of the trench.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120 volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and were used by employees did not have approved ground-fault circuit interrupters (GFCI) for personnel protection: (a) Park Range Construction, Inc. On or before December 13, 2012, the exposing employer did not ensure that the 120-volt outside wall receptacle outlet used to power operated hand tools was provided with approved ground-fault circuit interrupters (GFCI) for personnel protection. The condition exposed the employees to an electrical hazard.
