29 CFR 1926.651(b)(3):When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) 2445 South Quebec Street Denver, CO 80231: On or about March 12, 2025, the employer did not ensure utility locates were done prior to conducting excavation operations in the kitchen of the Eastern Star Masonic Retirement Campus, thus exposing employees to explosion, fire and electrical hazards.
