29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a handrail system along each unprotected side or edge: (a) KJO Construction, LLC @ 3334 Tejon Street, Denver, CO. On or before December 12, 2012, the exposing employer did not ensure that the interior stairways used by employees to gain access to the upper levels and the roof top of the building under construction, was equipped with a standard handrail system, to provide employees strong handhold. This condition exposed the employees to the hazard of falling when ascending and descending the stairways.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels was not protected by guardrails systems, safety net system, personal fall arrest system (PFAS), or other approved alternative fall protection systems: (a) KJO Construction, LLC @ 3334 Tejon Street, Denver, CO. On or before December 12, 2012, The exposing employer did not ensure that fall protection systems were provided while employees were engaged in residential construction activities 6 feet or more above lower levels in the following instances: (1) Employees had been crossing over from building to building under construction by using single OSB planking without guardrails installed. This condition exposed the employees to a fall hazard up to 9 feet walking platform to ground. (2) Employees had been performing activities in the first levels of the buildings under construction where open-sided floor(s) and unprotected window opening(s) had not been provided with guardrails systems to safeguard employees from falling from elevation. This condition exposed the employees to a fall hazard up to approximately 9 feet floor to ground.
