29 CFR 1910.305(g)(2)(ii):     Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.        a. At the workplace, the employer did not ensure that electrical wiring on the plastic extruder #4 near the blower motors, located in the rear of the warehouse were properly guarded from accidental contact, exposing employees to a fire hazard.     b. At the workplace, the employer did not ensure that electrical wiring on the plastic extruder #6 near the tooling, located in the rear of the warehouse were properly guarded from accidental contact, exposing employees to a fire hazard.    c. At the workplace, the employer did not ensure that electrical wiring on the plastic extruder #6 near the disconnect, located in the rear of the warehouse were properly guarded from accidental contact, exposing employees to a fire hazard.

Located at Flex Technologies in Midvale, Ohio:      a. On the 2012 OSHA 300 log, case number 4, the employer did not provide all information required in column F to include, but not limited to, the description of the injury or the object that directly caused the injury.    b.
