29 CFR 1910.134(d)(3)(ii): The respirator selected shall be appropriate for the chemical state and physical form of the contaminant: Facility: When selecting a respirator as protection during the clean-up of a spill of liquid acids, the employer selected a half-mask elastomeric respirator outfitted with HEPA cartridges. These cartridges are ineffective agaist the acid gas vapors that would be generated. The appropriate selection are those cartridges offering protection against acid gas vapors.

29 CFR 1910.141(a)(3)(iii): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, loose boards, and unnecessary holes and openings. Facility: In the Plating and Wastewater Treatment Areas the employer used wooden flooring to raise up the walking and working surfaces. This raised flooring had numerous sections of missing and broken floor boards, and open holes in the flooring. This created slip and tripping hazards and uneven and unstable walking surfaces for employees handling hazardous chemicals, and also in the event emergency egress is required.
