29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On December 18, 2012 at the jobsite located at 5215 Noland Road in Independence MO employees were exposed to a struck by hazard in that they were not wearing head protection while overhead work was taking place.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. On December 18, 2012 at teh jobsite located at 5215 Noland Rd in Independence MO employees were exposed to a fall hazard in that they were working from a mobile scaffold while standing on the guardrail.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On December 18, 2012 at the jobsite located at 5215 Noland Rd in Independence MO employees were exposed to a fall hazard in that they were owrking on top of a canopy roof approximately 18 feet above ground level without an approved form of fall protection.
