29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a commercial construction site located at 9600 North Oak Trafficway, Kansas City, Missouri: Employees were exposed to fall hazards. Employees were installing guttering working at height. Employees were exposed to a fall distance of 17 feet above the ground without the use of fall protection.

29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, were not withdrawn from service until repaired: At a construction site located at 9600 North Oak Trafficway, Kansas City, Missouri: Employees were exposed to fall hazards. Employees were using a Werner ladder to access the canopy above the gas station pumps. Employees were using the Werner extension ladder which had a gash in the metal side rail which could bend and cause the employee to fall.
