29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) The interlock switch on the tool change door for the Amada Vipros 367 turret was damaged and missing the interlock key. b) The point of operation on the Accur-press brake press was not guarded, allowing operators to access the area during the dangerous downward motion of the press. Abatement Certification and Abatement Documentation Required.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) On or about December 28, 2012, the control panel door was left open on the Haeger insertion machine for cooling purposes, exposing the operator to electrical hazards. Abatement Certification and Abatement Documentation Required.
