29 CFR 1910.217(c)(2)(i)(d): It shall utilize fasteners not readily removable by operator, so as to minimize the possibility of misuse or removal of essential parts; a. Located in the tow hook room, employees were required to operate mechanical power press 682 in order to bend metal. On the back of the power press, there was an expanded metal guard that was hinged but did not prevent employees from the point-of-operation of the power press due to a pin that was used to secure the guard that was readily removable, thereby exposing the employees to the movable parts of the press.

29 CFR 1910.219(h)(1): All projecting keys, setscrews, and other projections in revolving parts shall be removed or made flush or guarded by metal cover. This subparagraph does not apply to keys or setscrews within gear or sprocket casings or other enclosures, nor to keys, setscrews, or oilcups in hubs of pulleys less than twenty (20) inches in diameter where they are within the plane of the rim of the pulley. a. Located in the tow hook room, there was a conveyor that moved cold form tow hooks from mechanical power press 682. The conveyor had a rotating shaft that protrued from under the conveyor's belt that had setscrews that were not made flush or guarded, thereby exposing the employees to being pulled in to the rotating shaft.

Located in the tow hook room, employees were operating a conveyor that moved automotive tow hook parts from mechanical power press 682. Attached to the bottom of conveyor 521, there was drive motor and a conveyor stop button that had a flexible cord that was used in lieu of approved fixed wiring, thereby exposing the employees to an electrical shock and/or fire hazard.
