29 CFR 1910.217(c)(3): Mechanical Power Presses. Safeguarding the point of operation - Point of operation devices.    29 CFR 1910.217(c)(3)(iv):  The pull-out device shall protect the operator as specified in paragraph (c)(3)(i)(b) of this section, and shall include attachments for each of the operator's hands.    29 CFR 1910.217(c)(3)(iv)(b): Attachments shall be adjusted to prevent the operator from reaching into the point of operation or to withdraw the operator's hands from the point of operation before the dies close.    a. On or about September 26, 2014, while operating the Bliss 75 ton press known as Press 269 an employee suffered an injury while running the bracket rod parts 81813-B. The operator must hand feed parts into the point of operation. The point of operation safeguarding device known as a pullback was not properly adjusted to withdraw the operators hands from the machines danger zone before the dies closed resulting in the partial finger amputation.

Safeguarding the point of operation - Point of operation devices.    29 CFR 1910.217(c)(3)(iv):  The pull-out device shall protect the operator as specified in paragraph (c)(3)(i)(b) of this section, and shall include attachments for each of the operator's hands.    29 CFR 1910.217(c)(3)(iv)(d):   Each pull-out device in use shall be visually inspected and checked for proper adjustment at the start of each operator shift, following a new die set-up, and when operators are changed. Necessary maintenance or repair or both shall be performed and completed before the press is operated. Records of inspections and maintenance shall be kept in accordance with paragraph (e) of this section.     a. On or about September 26, 2014, while operating the Bliss 75 ton press known as Press 269 an employee suffered an injury while running the bracket rod parts 81813-B. The operator must hand feed parts into the point of operation. The point of operation safeguarding device known as a pullback was not properly adjusted to withdraw the operators hands from the machines danger zone before the dies closed resulting in the partial finger amputation. Prior to the injury, the setup man repaired the die by replacing the punch and ran parts to test the repair, without checking the pullback for proper adjustment prior to the operator change over.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    For the calendar year 2014 OSHA 300 log, the employer had not included all the information in column F as required for the following entries:    a.) Case no. 01 did not include the object/substance that directly injured the employee,    b.) Case no. 02 did not include the part of the body affected and the object/substance that directly injured the employee,    c.) Case no. 03 did not include the object/substance that directly injured the employee,     d.) Case no. 04 did not include the object/substance that directly injured the employee, and    e.) Case no. 05 did not include the object/substance that directly injured the employee.     Midwest Fabricating Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1904.29(b)(1), which was contained in OSHA inspection number 805621, citation number 2, item number 1 and was affirmed as a final order on May 13, 2013, with respect to a workplace located at 313 Johns Street, Amanda, Ohio 43102.
