29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.      On or about January 2, 2013, on the north side, employees engaged in framing activities on a residential construction site were not protected from head injuries.  The employer did not provide head protection exposing employees to struck-by falling and flying objects.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe.      On or about January 2, 2013, on the north side, employees engaged in framing activities on a residential construction site utilized a Makita skill saw with the lower guard wedged in the upright position with a wooden wedge.  The employees were exposed to amputation hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.      On or about January 2, 2013, on the north side, employees engaged in framing activities on a residential construction site utilized electrical equipment without strain relief devices exposing employees to electrical hazards:      a)	 Extension cord without a strain relief device utilized to energize a Makita skill saw, and    b)	Makita skill saw power cord without a strain relief device utilized to cut framing materials.
