29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: (a) Herod Industries Inc. at Bulldog 5 near Craig, CO 81626: The employer did not ensure that grounding and bonding methods were used to control electrostatic ignition sources when off-loading produced water containing hydrocarbons and hydrocarbon vapors. This condition exposed employees to explosion and fire hazards of flammable or combustible liquids and gases. (b) Herod Industries Inc. at Bulldog 5 near Craig, CO 81626: The employer did not take adequate precautions to control ignition sources including heat and open flame, in that a propane torch was used to heat a frozen transfer line connecting a vacuum truck tank and a stationary storage tank which resulted in an explosion due to the ignition of vapors within one or both tanks. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

This condition exposed employees to explosion and fire hazards of flammable or combustible liquids and vapors. (b) Herod Industries Inc. at Bulldog 5 near Craig, CO 81626: The employer did not ensure that the portable vacuum truck tank containing produced water, was labeled, tagged or marked to identify the contents. This condition exposed employees to explosion and fire hazards of flammable or combustible liquids and vapors.
