29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which, when subjected to heat, might produce flammable or toxic vapors: a) Herod Industries Inc. at 1205 Rose Street, Craig, CO 81625: On or about July 5, 2013, the employer did not ensure that a 130 barel (5,460 gallons) vacuum tractor trailer (Fluidmaster serial number 1M9AV40224N556041) was cleaned and purged of hydrocarbons prior to cutting off an angle iron hose hanger that had been welded to the tank. Condition exposed employees to the explosion hazards.
