29 CFR 1904.41(b)(2): The OSHA Annual Survey Form was not completed and sent to OSHA within 30 calendar days, or within the time stated in the Survey Form, whichever is later. San Mateo Inc. failed to complete and send to OSHA, or its agent, the "OSHA Occupational Injury and Illness Data Collection Form, 2009" within 30 days of the date. San Mateo Inc. received the Form, which was mailed to San Mateo Inc. on June 20, 2012. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
