29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) All departments throughout the plant. On or about March 26, 2013, employer did not establish and implement lockout/tagout program for employees to receive training and utilize formal and specific lockout/tagout procedures for energy isolation on the identified machines where the hazard of unexpected energization could occur.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone where hazards such as those created by the rotating and moving parts during the operating cycle were present:    (a) North east wall; sheet metal dept. On or about March 26, 2013, employees were exposed to amputation and caught-on hazards on the Red Amada Turret Pega-345 King and Blue Amada Turret Pega 30T where the protective plexi glass shields located on the interlocked doors were damaged exposing employees to point of operation hazards.    (b) South west wall; grinding and deburring dept. On or about March 26, 2013, employees were exposed to caught-on/in hazards on the Woodtek D.06 horizontal belt sander where the enclosure guards were damaged and had holes on the top covers.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about March 26, 2013, DeWalt D28402 portable handheld angle grinder was used by an employee to smooth surfaces of welded parts exposing employee to cuts and lacerations hazards where the grinder was not equipped with an adequate safety guard.
