29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) D.C. Fabricators, Main Fabrication floor: Employees were exposed to fall hazards when using scissors lifts that have guardrails removed and without using a lanyard or any other means of fall protection while welding pieces at a height of more than 6 feet to the ground surface below, observed on or about 1/14/2013. b) D.C. Fabricators, Main Fabrication floor: Employees were exposed to fall hazards while welding on top of the manufactured pieces such as steam condensers and heat exchangers at a height of more than 4 feet to the ground surface below. The employees do not use fall arrest systems or other means of fall protection , occurred on or about 2/05/2013.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) DC Fabricators - Small fab shop: The path to ground was not effective on the welding manipulator (Aronson Machine. The ground conductor cable had a broken sheathing and the ground wire was making contact with the machine, observed on or about 01/14/2013.
