29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Jobsite, Lot Number 212, Parkview Boulevard Pittsburgh, PA 15217: Observed on or about January 8, 2013, employees engaged in roofing activities on a two story residential dwelling were not protected from falling by guardrail systems, safety net systems, or personal fall protection systems. The slope of the roof ranged from 4 in 12 (vertical to horizontal) to 9 in 12. The employees were exposed to a fall hazard of approximately 16 feet to ground below.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: a) Jobsite, Lot Number 212, Parkview Boulevard Pittsburgh, PA 15217: On or about January 08, 2013, the employer did not retrain employees who were observed performing roofing work on a two story residential dwelling without the use of fall protection. The employees were exposed to a fall hazard of approximately 16 feet to the ground below.
