29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Worksite, 1567 Park View Blvd, Pittsburgh, On or about October 7, 2015 - Employees were working and walking on and near the edge of a residential roof while constructing a roof top deck. Employees were not protected from falling and exposed to an approximately 8-10 feet to ground fall hazard.
