29 CFR 1910.119(d)(2)(i)(A): The process safety information (PSI) pertaining to the technology of the process did not include a block flow diagram that contained accurate information: a) Nordic Logistics and Warehousing: The employer's current PSI (Process Safety Information) contained a block flow diagram that was not current and updated to show an accurate flow of ammonia refrigeration within all major vessels. A blast freeze area was indicated on the blow flow diagram that was no longer in operation. Employees were exposed to materials such as, but not limited to, ammonia while performing refrigeration repairs and maintenance.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) Engine Room #1: The employer failed to ensure that an eyewash/shower unit was located in the interior area of the refrigeration room where compressor units, piping and other equipment were located. Employees were exposed to materials such as, but not limited to, ammonia while performing refrigeration maintenance on the ammonia system. In accordance with ANSI/IIAR Standard 2-2008, Section 13.3.1.4 and IIAR Bulletin 109, Section 4.10.10, 1997 edition; OSHA 29 CFR 1910.151(c).
