29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) On or about 1/09/2013, two employees were on an aerial lift S/N 889214 without being attached to the basket. b) On or about 1/09/2013, two employees were on an aerial lift S/N 0300089257 without being attached to the basket. c) On or about 1/09/2013, two employees were on an aerial lift S/N 0300107896 without being attached to the basket. NOTE: PI Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, which was contained in OSHA inspection number 312153471, citation number 1, item number 1 and was affirmed as a final order on March 02, 2009, with respect to a workplace located at 900 S ocean Drive, Boca Raton, FL 33432.
