29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite. This requirement does not apply to the integral fuel tanks of motor vehicles.    On or about 01/09/2013 at the Doubletree Hotel renovation project, employees were using a 20 pound rated propane tank and 3 gallons of flammable QuickPrime Plus adhesive on a roof with no fire extinguisher within 50 feet.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about 01/09/2013 at the Doubletree Hotel renovation project, the employees were not trained on fall protection requirements and usage of fall protection while working on an ice covered roof.

29 CFR 1926.150(c)(1)(i):  Travel distance from any point of the protected area to the nearest fire extinguisher exceeded 100 feet:    On or about 01/09/2013 at the Doubletree Hotel renovation project, employees were working on a roof using a torch, propane, and flammable roof adhesive and the nearest fire extinguisher was located on the ground in the company truck located approximately 250 feet travel distance, to and from.

On or about 01/09/2013 at the Doubletree Hotel renovation project, employees were exposed to fire hazards while working with flammable liquids/gas such as QuickPrime Plus adhesives and propane.  The employees did not know the fire extinguisher requirements or of not working within 50 feet of flammables while using a torch.

29 CFR 1926.502(h)(1):  The employer did not did not designate a competent person to monitor the safety of other employees and/or did not ensure that the safety monitor complied with the requirements listed in (h)(1)(i) through (h)(1)(v) of this section:    On or about 01/09/2013 at the Doubletree Hotel renovation project, employees were exposed to 12 foot fall hazard while working from an ice covered roof with the safety monitor system in use and the employees did not know the safety monitor duties or responsibilities.
