29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):    (a)  On or about 4 January, 2013, the employer did not ensure that employees were protected from cave-in injuries while replacing a waterline in a trench in excess of five feet deep without the benefit of a protective system in place.    Hudick Excavating, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent Standard, 29 CFR 1926(a)(1), which is contained in OSHA Inspection Number 313722845, Citation 2, Item 1, issued on 15 October, 2010.  With respect to a workplace located at Easter and Lima, Centennial, CO 80110.  This citation became a Final Order on 2 November, 2010.
