29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe:    (a)  On or about May 26, 2015, employees were exposed to injury in that, rigging equipment, including two Crosby brand shackles, was not inspected prior to use to ensure that it was safe for use.  Employees were seriously injured when one such shackle used to lift a GAR-BRO concrete bucket, came loose thus resulting in the bucket falling and striking the employees.
