29 CFR 1910.107(b)(9):     A clear space of not less than 3 feet on all sides was not kept free from storage or combustible construction:    (a)  The employer did not ensure that the "red glue" booth was maintained free of combustible materials such as but not limited to a drum of red glue solvent-based adhesive and wood materials.  Employees were exposed to fire hazards.     (b)  The employer did not ensure the spray booth in the hard wood area was free of combustible materials such as but not limited to wood materials and mineral spirits. Employees were exposed to fire hazards.

29 CFR 1910.213(c)(1):     Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a) In the production area, the employer did not ensure effective blade guarding was utilized during the operation of the Powermatic table saw(s) [Model: 66, Serial: 8866747]. The hinged guards were flipped open to cut stock and then closed to protect the blade when in the off position. Employees were exposed to amputations when cutting stock.

29 CFR 1910.213(r)(4):     Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation on the following chop saw:    (a) The employer did not ensure the Whirlwind chop saw (model & serial numbers unknown) had sufficient point of operation guarding in that employees' fingers and/or hands could be trapped under the hood guard and contact the saw blade. Employees were exposed to amputation hazards.

29 CFR 1910.134(c)(2)(ii):     The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    (a) The employer did not ensure that all employee(s) who wore a tight-fitting respirator, whether personally owned or issued by the company, were medically evaluated prior to using it in the spray booths.
