29 CFR 1910.332(b)(1):  Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments:              (a)  Phillips Mfg., 5578 State Route 61N, Shelby, Ohio:  On or about January 8, 2013, the employer did not ensure that maintenance personnel working on an energized or potentially energized 480 volts, 3-phase circuit, were familiar with or trained on how to protect themselves from electric shock, arc-flash, and arc-blast hazards from energized electrical parts and equipment.  Personnel were not aware of safe approach distances, were not aware of the need for electrical personal protective equipment (PPE), and were not knowledgeable in the selection of proper PPE to protect from electric shock, arc flash, and arc explosion.  Employees used a Fluke model 87 multi-meter to verify an energized circuit on a Havers cutting unit.

29 CFR 1910.333(a):  Safety related work practices were not employed to prevent electrical shock or other injuries resulting from either direct or indirect contacts, when work was performed near or on equipment or circuits which were or could be energized:             (a)  Phillips Mfg., 5578 State route 61N, Shelby, Ohio:  On or about January 8, 2013, and on a continuing basis, the employer did not ensure that maintenance personnel working on 480 volts, 3-phase circuits followed safe work practices.  The employer did not develop and implement electrical safe work practices addressing the hazards of electrical shock, arc flash, and arc explosions.  Maintenance personnel were not aware of safe approach distances and were not aware of the need for electrical personal protective equipment.   Employee used a Fluke model 87V multi-meter to verify an energized circuit on a Havers cutting unit.

29 CFR 1910.335(a)(1)(i):   The employer did not ensure that employees working in areas where there are potential electrical hazards were provided with, and use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.           (a)  Phillips Mfg., 5578 State route 61N, Shelby, Ohio:  On or about January 8, 2013, the employer did not ensure that maintenance personnel working on an energized or potentially energized 480 volts, 3-phase circuit used arc flash and electrocution protection, such as but not limited to, arc rated clothing and voltage rated protective gloves.  It was an on-going practice for maintenance personnel to check the voltage on energized and potentially energized circuits with a Fluke model 87 multi-meter.
