Phillips Mfg. & Tower Co. dba Shelby Welded Tube located at 5578 St. Rt. 61N Shelby, OH; On or about July 17, 2014, the employer did not assure employees were protected while operating the Eagle Tube Bender (Serial #96-934) and Eagle Tube Bender (Serial #96-938). Employees that operated the Eagle Tube Bender to bend metal parts were exposed to amputation injuries in that the employer did not assure pinch points were guarded.
