29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Phillips Mfg. & Tower Co. dba Shelby Welded Tube located at 5578 St. Rt. 61N Shelby, OH; On or about July 17, 2014, the employer did not assure employees were protected while operating the Eagle Tube Bender (Serial #96-934) and Eagle Tube Bender (Serial #96-938). Employees that operated the Eagle Tube Bender to bend metal parts were exposed to amputation injuries in that the employer did not assure pinch points were guarded.
