29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    a. On the concrete pad directly below the area employees were installing roofing trusses, employees did not have hard hats, thereby being exposed to a struck by hazard.

29 CFR 1926.451(b)(1)(i):     Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).    a. On the mobile scaffolds where employees were working from, the platforms were not fully planked between the uprights, thereby exposing employees to a fall hazard.

29 CFR 1926.451(g)(4)(i):     Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a. On the scaffolds where employees were working from, guardrails were not installed, thereby exposing employees to a fall hazard.

29 CFR 1926.452(w)(1):     Scaffolds shall be braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections shall be secured.    a. On the north end of the building where employees were working from a mobile scaffold, cross braces were not installed on both sides of the middle or top sections of a mobile scaffold, thereby exposing employees to a fall hazard.

29 CFR 1926.501(b)(1):     "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On the top plate of the building where an employee was installing roof trusses, fall protection was not provided, thereby exposing employees to a fall hazard.    b. On the trusses of the building where employees were bracing the roof trusses, fall protection was not provided, thereby exposing employees to a fall hazard.
