29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a. 1 College Blvd Paterson NJ:  Employees working without hardhats were exposed to a struck by hazard from a  steel debris container that was being hoisted over the their heads by a crane, on or about 01/10/2013.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:          a. 1 College Blvd, Paterson NJ:  Employees were exposed to a fall hazard of approximately 30 feet while using a Werner extension ladder that did not extend 3 feet above the roof landing surface, on or about 01/10/2013.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system    a. 1 College Blvd Paterson NJ:  Employees were exposed to a fall hazard of approximately 50 feet while engaged in roofing work without a fall protection system in place, on or about 01/10/2013.     Safeway Contracting Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(10): which was contained in OSHA Inspection Number 312866254, Citation # 1, Item # 4, issued on 8/03/2010 to a workplace located at 365 Boulevard, Hasbrouck Heights, NJ 07604 (Hasbrouck Heights Middle and High School) which became a final order on 9/08/2010.
