29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about January 10, 2013, south side of the construction site, an employee was exposed to a fall hazard of approximately 9 feet while installing Tyvek from the roof with a 12:12 pitch of a residential building.

29 CFR 1926.503(a)(2)(i): Employees were not trained, as necessary, by a competent person qualified to recognize the nature of fall hazards in the work area. On or about January 10, 2013, south side of the construction site, an employee was exposed to a fall hazard of approximately 9 feet while installing Tyvek from the roof of a residential building with 12:12 pitch. The employer did not provide training to the employee on recognizing and correcting hazards associated with falls.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about January 10, 2013, south side of the construction site, an employee, who was installing Tyvek insulation from a residential roof, utilized an extension ladder that did not extend 3 feet above the upper landing, to access and egress the roof. The employee was exposed to a fall of up to 9 feet to the ground below.
