(c)    Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, SW Pod: A ladder was not provided for employees to use to access the scaffold, employees of a subcontractor under the supervison of Tekcon Construction Inc., climbed the cross braces and end frames to access the scaffold's walking/working surface exposing themselves to fall hazards, observed on or about  1/2/13.      (d)   Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, Front of Building Side Entrance was on: The scaffold system was not provided with a ladder.  Employees of a subcontractor under the supervison of Tekcon Construction Inc., climbed the cross braces and end frames  to access the scaffold system exposing themselves to falls of approximately 20' to the ground below, observed on or about 1/3/13.      (e)   Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, and SW Wing: The access ladder only went up to the second platform, requireing employees of a subcontractor under the supervison of Tekcon Construction Inc., to climb the cross braces and end frames to access the third level of the scaffold system exposing themselves to falls of approximately 20' to the ground below, observed on or about 1/5/13.      (f)     Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, POD Back Deck:  A ladder was not provided for employees to use to access the side bracket or the top level of the scaffold, therefore requiring employees of a subcontractor under the supervison of Tekcon Construction Inc., to climb the cross braces and end frames to access the scaffold's walking/working surface.  Therefore, exposing themselves to falls of approximately 20' to the ground below, observed on or about 1/31/13.          (g)   Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, Exterior Pool Room Wall:  The ladder was only elevated to the 1st level of the scaffold system.  Employees of a subcontractor under the supervison of Tekcon Construction Inc., were working on the second and third levels of the scaffold system and used the cross braces to access those levels exposing themselves to falls of approximately 20' to the ground below, observed on or about 2/22/13.
