29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: Building 559A: On or about January 7, 2013 and at times prior thereto, the fan blades with the lower blade periphery at 2 feet, 4 inches were not guarded on two (2) floor-supported, Taskmaster unit heaters. This condition exposed employees to laceration hazards from contact with the rotating metal blades.

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: Provost Marshal Division: On or about January 7, 2013, the employer failed to report to OSHA a work-related death which occurred on December 27, 2012. This condition hindered the ability of the area office in completing a thorough investigation of the circumstances leading to the fatality.
