On or about January 10, 2013, on the east side, employees engaged in masonry work on a residential construction site, utilized a Target masonry saw with an extension cord spliced into the motor, exposing employees to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.      On or about January 10, 2013, on the west side, employees engaged in masonry work on a residential construction site, utilized Target masonry saw.  The masonry saw power cord did not have a strain relief device to prevent pull from the terminal screws exposing employees to electrical hazards.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.       On or about January 10, 2013, on the east side, employees were engaged in masonry work from a tubular welded frame scaffold that was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.      On or about January 10, 2013, on the east side, employees engaged in masonry activities on a residential construction site worked from a tubular welded frame scaffold.  The employees utilized the scaffold frame and cross braces to assess and egress the second level exposing employees to fall hazards up to 14 feet.
